Since its inception in the summer of 2004, Community PHIND has been a product of students and faculty of the Johns Hopkins Bloomberg School of Public Health and others interested in the study of community health, especially the role of the environment, community-level factors and population-based measures on local area health. Work to provide community-level Public Health Indicators and Nationwide Data is an ongoing, collaborative and creative effort.

Special thanks to Shirley Crow, website designer, and Dan Gudgel, for website updates and the development of the original interactive community environmental reports facility. Graphic design of local area geography is courtesy of Chih-Hung Cho and Shu-Chi Su. The model of social determinants of health and well-being, communityPHIND.net logo, links, references, organizations, resources, and glossary were originated by Alena J. Groopman.

Community-level data and conceptual aggregation of indicators were contributed by many individuals: Melissa Brower, Roger Chung, Lisa Gallicchio, Alena Groopman, Marilyn Metzler, Dave Moriarty, Sarah Shaio, Jennifer Stanley, Yin Wu, and Matt Zack. Encouragement and feedback were provided by the Social Determinants of Health Committee (CDC) and the Maryland Department of the Environment, Environmental Justice Health Indicators Subcommittee.

Support for participation in community health investigations has been provided in part, by the Maryland Cigarette Restitution Fund (JHMI, MD DHMH), the Center for Excellence in Environmental Public Health (CDC), the Center in Urban Environmental Health (NIEHS), the Summer Diversity Internship Program (JHSPH), and the Institutional National Research Service Award (NCI).
